The 3 S's of Scar Care: Maximizing Your Surgical Results
One of the most common questions I receive from patients after surgery is: "How can I make sure my scars heal well?" It's an excellent question, because while scars are a natural part of any surgical procedure, how they heal can significantly impact your final results.
Understanding Scar Formation
Scar formation is a complex and somewhat unpredictable process. The quality of your scar depends on three main factors:
- Your genetics - Some people naturally heal with minimal scarring, while others are more prone to thicker or more visible scars
- Surgical technique - Precise incision placement and meticulous closure techniques make a significant difference
- Post-operative scar care - This is the factor YOU control
Since post-operative care is entirely within your control, that's where I want to focus. The good news? Effective scar care doesn't have to be complicated.
The 3 S's of Optimal Scar Care
I teach all my patients to remember the "3 S's" of scar care. These three simple practices can dramatically improve your scar quality and help you achieve the best possible aesthetic outcome.
1. Sun Avoidance
Why it matters: Fresh scars are particularly vulnerable to hyperpigmentation. During the first year of healing, scar tissue readily absorbs UV radiation, which can cause permanent darkening and make scars much more visible.
What to do:
- Apply broad-spectrum SPF 30+ sunscreen to healing scars whenever they'll be exposed to sunlight
- Reapply every two hours when outdoors
- Wear protective clothing, such as a hat, when possible
- Consider keeping scars covered during peak sun hours
Think of sunscreen and hats as your scar's best friends for that critical first year of healing. This simple habit can mean the difference between a faint, barely visible scar and one that stands out prominently.
2. Scar Massage
Why it matters: Scar massage helps soften and break down scar tissue, preventing thick, raised scars and promoting a smoother, flatter contour. This technique improves circulation to the area and helps remodel collagen fibers in a more organized pattern.
When to start: Begin scar massage at two weeks post-surgery, once your incisions are fully closed and your surgeon has given you clearance.
How to do it:
- Use gentle, circular motions with your fingertips
- Apply firm but comfortable pressure
- Massage for 5-10 minutes, 2-3 times daily
- You can use a mild, fragrance-free moisturizer to reduce friction
- While many creams have been posited, none have been shown to be superior so we recommend you opt for a hypoallergenic, cost-effective moisturizer such as Vaseline or Aquaphor
Consistency is key with scar massage. Make it part of your daily routine for the best results.
3. Silicone Sheeting
Why it matters: Silicone sheeting has been extensively studied and proven to improve scar quality. It works through two mechanisms: hydrating the scar tissue and providing mechanical support to the healing wound. Both effects help minimize scar formation and reduce the appearance of existing scars.
What to know:
- Medical-grade silicone sheets or gels are available over-the-counter
- Sheets should be worn for 23+ hours daily for optimal results
- Treatment typically continues for 2-3 months or longer
- Silicone gel is an alternative for areas where sheets don't adhere well
Silicone products create an ideal healing environment that encourages your body to produce flatter, softer, less noticeable scars.
