WHO WILL FOLLOW THIS NOTICE?
This notice describes the privacy practices of Conlon Plastic Surgery and applies to all staff
members who handle your health information.
OUR PLEDGE REGARDING YOUR PROTECTED HEALTH INFORMATION
Conlon Plastic Surgery recognizes that your health information is personal. We are dedicated to protecting your medical information, maintaining confidentiality, and ensuring its security while providing high-quality care. This notice covers all records relating to your medical treatment created or received by Conlon Plastic Surgery.
Your other healthcare providers (such as hospitals, laboratories, or specialists) may have different policies regarding your medical information. This notice describes the ways Conlon Plastic Surgery may use and share your protected health information (PHI), which includes demographic details and health data that could identify you and relate to your health and healthcare services. It also explains your rights and our responsibilities concerning your PHI. Under law, we are required to:

USES AND DISCLOSURES FOR TREATMENT, PAYMENT, AND HEALTH CARE
OPERATIONS
By becoming our patient, you consent to Conlon Plastic Surgery using your protected health information for treatment, billing, and healthcare operations, often called "TPO".
- Treatment: We may use and share your PHI with medical staff involved in your care,or with other providers to coordinate treatment.
- Payment: We may submit your PHI to insurance companies to process claims andsecure payment for services.
- Health Care Operations: We might review our procedures to improve services orshare PHI with auditors to maintain licensing.
Other Uses and Disclosures Without Additional Authorization
The law permits some uses and disclosures of your PHI without your explicit permission, including:
- Licensure Activities: For certification or licensing audits
- Appointment Reminders: Contact you about appointments, with confidentiality options available—please contact us at hello@conlon.md if needed
- Family or Others Involved in Care: We may share PHI with family or friends directly involved in your care or responsible for your treatment decisions unless you object
- Emergency Situations: Use or share PHI when immediate treatment is necessary
- Health Benefits and Services: To inform you about relevant health services orbenefits
- Legal Obligation: Disclosure required by law, such as for communicable diseases,court orders, or law enforcement requests
- Public Health and Safety: Reporting contagious diseases, abuse, neglect, or forsafety reasons
- Research: When authorized through approved protocols
- Legal Disputes: Disclosure in response to legal requests such as subpoenas
- Law Enforcement: When legally mandated or necessary for security or safety
- Death and Organ Donation: Disclosures to coroners, funeral homes, or for organdonation
Your Rights Regarding Your PHI
- Access and Copy: You may request to view or obtain a copy of your medical
records by submitting a written request. - Restrictions: You can request that we limit how we use or share your PHI; we will
review and respond in writing. - Confidential Communications: Ask to receive communications in ways or places
that protect your privacy. - Amendments: Request changes to your records if you believe they’re inaccurate or
incomplete. - Accounting of Disclosures: Ask for a list of disclosures made outside treatment,
billing, or operations over the past six years. - Paper Copy of this Notice: You can request a paper version at any time, or view it

Changes to This Notice
We may update this notice periodically. The latest version will be posted on our website with an effective date.

If you believe your privacy rights have been violated, or you have concerns, you may file a
complaint with our staff or with the Department of Health and Human Services.
