Implant-based breast reconstruction is a surgical procedure that uses breast implants to recreate the shape and look of breasts after one or both breasts are removed due to cancer. Implants are silicone shells filled with silicone gel–to closely approximate the consistency of breast tissue. This type of reconstruction can be a good option for many women.

Implant reconstruction may be recommended for women who have sufficient healthy skin and tissue to cover an implant after mastectomy. Direct-to-implant breast reconstruction is a good option for women who want to be approximately the same size after their mastectomy and reconstruction. Patients that want to be significantly smaller after their mastectomy may be candidates for direct-to-implant reconstruction or be recommended for a breast reduction in advance of the mastectomy. Patients that want to be larger after their mastectomy will typically need an intermediate stage with a temporary implant, known as a tissue expander, to increase the soft tissue of the breast. The nuances of your anatomy will guide your plastic surgeon and surgical oncologist to a customized treatment plan for you if you are a good candidate for implant-based reconstruction.

- Consultation
You'll meet with a plastic surgeon to discuss your goals and treatment options. They will examine you and will take measurements and photos for your medical record. Your plastic surgeon will also speak with your breast surgeon to discuss the likely treatment course for your cancer care, as that will dictate the options available. - Mastectomy
The breast surgeon will remove breast tissue, and in some cases, the nipple and areola. Lymph nodes may also be removed. - Inserting implant
A breast implant can be placed at the time of the mastectomy if your anatomy–the thickness and blood flow to your breast skin–permits immediate placement. This decision will be made intra-operatively so all options will be discussed during consultation. If there is insufficient skin to cover an implant, a balloon-like tissue expander is placed. Over several weeks or months, saline is gradually introduced to stretch the skin. Exchanging for an implant then happens at a later date. - Creating the nipple and areola
If the nipple and areola were removed during mastectomy, they can be recreated in a separate procedure after the initial reconstruction.
